226-0609 and schedule a consultation! When
you come in and meet with one of our
providers we will be able to apply for you
and give you an answer on the spot.
If you prefer to pay by cash or check,
payment in full is due at your pre-surgical
visit, two weeks prior to surgery. We accept
personal and certified checks, but not business
checks.
